If you cannot resolve an issue within your home with the help of the house manager, please submit a grievance form using the button below. This will be sent to the program director and also be visible to the operators/owners.
If the grievant feels that the matter has not been addressed sufficiently within FSL, a formal grievance can be filed with the association responsible for oversight (AzRHA) using the button below.
Another option for filing a grievance is through AZDHS. After you click the button to access the form, you will need to search your sober living facility by address. Select the correct facility and then follow the prompts on the website.
